New Patient Information

Your First Visit

During your first visit to our Grand Island dentistry office, we’ll review your medical and dental histories, take X-rays of your mouth, check for cavities, gum disease, and anomalies, and offer information about the best way to care for your teeth at home. In addition, we’ll clean and polish your teeth.

After your checkup and teeth cleaning are finished, Dr. Stec will meet with you to discuss your concerns and to co-create a viable treatment plan for you. We’ll look at your restoration and cosmetic options, as well as a plan for future preventive care.

When you arrive for your appointment, please bring your completed patient forms, your insurance information (if applicable), your government ID, and your preferred method of payment.

New Patient Forms

We offer our patient forms as downloadable PDF files. Simply open, print, and fill out in the convenience of your home. We look forward to meeting you and your family at your first appointment with Dr. Stec!
